Request an Appointment

Appointment times vary for each physician. Most physicians begin at 8:00 am and schedule the last appointment at 3:30 pm. Please contact us at 966-0111 to schedule an appointment.

When scheduling an appointment, please have your current insurance information and referral number ready. If you need to cancel or reschedule an appointment, please call us so that we may give that time to another patient.

For follow-up exams, please inform us of any changes to your health history, medications or insurance coverage.

Insurance and Billing

Insurance information:

We accept most major insurance carriers and participate in workers’ compensation compensation.

Financial Policy:

We accept cash, check, Visa, MasterCard, and most insurance plans. We honor all group insurance claims and by law, we must file Medicare claims.

If you are involved in a lawsuit that affects the payment of our services, you are responsible for payment of our regular fees at the time of service, unless other arrangements are made.
